To pass identification and accreditation, the buyer must provide the Exchange in paper or electronic form (with QES):
An application for accreditation certified by signature or QES of this person (Appendix 5 to the Regulation);
Copy of the passport (or other valid identity document) certified by signature or QES of this person;
Copy of the certificate on the assignment of an identification code (registration number of the taxpayer’s account card) certified by the signature or QES of this person;
If the buyer-individual is represented by another person, then a power of attorney is provided (Appendix 4 to the Regulation).
To provide the Exchange with the documents required for identification and accreditation in electronic form (using QES), you can use the “Accreditation Online” service. In order to perform this action please DOWNLOAD and install the installation package of the ITS “UICE-online” (includes the plug-in Microsoft Silverlight), open the page “Accreditation online” (the link of the “Accreditation online” is on the front page of the ITS “UICE-online” under the user authorization form).
Certified Application for accreditation (Appendix 5 to the Regulation);
Certified copy of the Statute (or other document confirming the existence of a registered Statute);
Certified copy of an extract from the National State Registry of Ukrainian Enterprises and Organizations (or another document confirming the registration of a legal entity);
Certified copy of the document in the form of a certificate or information from EDRPOU (if any);
Certified copy of the Extract from the register of VAT payers (in case of registration of a legal entity as a VAT payer);
Certified copy of the Extract from the register of single tax payers (in case of registration of a legal entity as a single tax payer);
Certified copy of the document confirming the powers of the head of the legal entity;
Certified Copies of the passport and certificate of assignment of the identification code (registration number of the taxpayer’s registration card) of the head of the legal entity;
• Certified copy of the power of attorney (Appendix 4 to the Regulation) or other document for an authorized person who represents to a legal entity and determines its powers within the auction (the right to participate in the auction, perform operations in the ITS “UICE-online”, sign documents following the results auction, etc.);
Certified copies of passport and certificate of assignment of an identification code (registration number of a taxpayer account) of an authorized person.
